I/We declare that all company vehicles which are used in respect of funerals fully comply with insurance and motor tax requirements and have successfully passed their N.C.T.s.
I/We also confirm that all clinical waste is disposed of by a registered contractor.
I/We further declare that Collier's Funeral Services Ltd. understand that the facilities as well as my/our Health and Safety Statement are subject to inspection of the Monitoring Committee from the Irish Association of Funeral Directors and that this Certificate must be renewed every two years to ensure continued participation in this programme.
